We invite you to share your personal story of how neurocomplex conditions have impacted you. Please complete the story submission template below, and make sure to download, review, and sign our Story and Media Release Agreement.
Important: Story submissions are not anonymous. You choose whether your story is considered for public sharing (name, first name, pseudonym, or anonymous) and how you want to engage in public advocacy content in the future (podcast, short film, or possible ConcertStories project).
